Job Summary:

Under the direction of the WOCN, primary role will include, but not be limited to, the direct care of patient and family of patients with gastrointestinal and genitourinary stomas, gastrointestinal and genitourinary continence disorders, acute/chronic wounds and fistulas. Direct care may encompass the complete care continuum and include assessment, testing, planning of care, intervention(s) and on-going re-evaluation of patients.

Essential Responsibilities:



  • Under the direction of a WOCN, may provide follow-up and interventions to patients diagnosed with ostomies, draining wounds, fistulae, or skin breakdown secondary to pressure, incontinence, venous hypertension, arterial insufficiency, trauma, surgery or diabetic neuropathy and other related skin issues.

  • Under the direction of a WOCN, may provide direct patient care counseling of individuals anticipating surgery which may result in the creation of a stoma, individualized instruction for those who have an abdominal stoma for the diversion of stool or urine, or assessment and treatment recommendations for the above stated skin breakdown conditions.

  • Assists patient through continuum of care for management of related issues.

  • Evaluates new and currently used products for ostomy, continence and wound care.

  • Makes recommendations to patients in obtaining related products.

  • Under the direction of a WOCN, may assess individual patient needs and goals, and provides educational programs to physicians, nursing, and other paramedical professionals on ostomy/wound care management and self-care education.

  • Assist the WOCN in conducting continuing educational programs for physicians and nursing personnel. This helps provide current standards of practice for ostomy patients.

  • Assist the WOCN in providing patient/family teaching and education regarding ostomy/wound care and self-management.

  • Consistently supports compliance and the Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures.

  • In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives. Basic Qualifications: Experience



  • Minimum one (1) year previous clinical nursing experience as a Registered Nurse.





Education

  • Bachelors degree required.
  • Acceptance into an approved, accredited Wound, Ostomy, Continence Nursing (WOCN) program.
License, Certification, Registration
  • Wound Ostomy Continence Nurse Certificate within 12 months of hire from Wound, Ostomy and Continence Nursing Certification Board OR Wound Ostomy Care Nurse Certificate within 12 months of hire from Wound, Ostomy and Continence Nursing Certification Board
  • Registered Nurse License (California)

  • Basic Life Support

Additional Requirements:

  • Demonstrated knowledge of pediatric and adult patient teaching principles.
  • Effective communication skills.
  • Fine motor dexterity.
